What You’ll Do
Provide comprehensive medical care by interviewing, examining, diagnosing, and treating patients.
Deliver patient-centered primary care with an emphasis on wellness, prevention, and early detection.
Educate patients and families on health conditions, treatment plans, and preventive care strategies.
Coordinate specialty care based on clinical assessment and patient needs.
Lead a team of providers and clinical staff in the management of a complex senior population to achieve high quality outcomes and guideline-based care.
Address goals of care and end of life issues with patients and their families or caregivers in a collaborative and respectful manner
Ensure accurate and timely medical documentation in the electronic medical record (EMR) that clearly captures and addresses all patient chronic and acute conditions
What You’ll Need
MD or DO from an accredited medical school.
Current, unrestricted medical license in the applicable state(s).
Board certified or board eligible in Internal Medicine or Family Practice (if board eligible, must become board certified within 12 months).
Active DEA license.
Commitment to quality care and health equity.
Passion for mentorship and team collaboration.
Satisfactory completion of a Tuberculosis (TB) test.
